A feedback form is a tool used to collect valuable insights and evaluations from participants about a specific session. It helps gather their opinions on various aspects of the session, such as content, delivery, and overall effectiveness. Below are the two types of feedback forms available on the platform:
After creating a participant group and session, a default feedback form with either 11 or 6 questions is automatically attached to your session based on the focus area or region.
To enhance flexibility, iECHO includes a form builder that allows organizations to customize and add questions to the feedback form as needed.
Types of Forms Available
1. 11-Question Feedback Form
This IRB-approved form is used globally in organizations, excluding India. In India, it is applicable only for programs in Oncology and Palliative Care.

2. 6-Question Feedback Form
This form is for programs in India, covering all focus areas except Oncology and Palliative Care.

It is recommended to avoid editing the standard feedback form. If you edit the existing form, it will not be reflected in the Feedback Dashboard. However, you can add additional questions as needed to gather more specific information.
1. Do not delete a question with a recorded response, as it can affect reporting.
2. Editing a question with a recorded response is not available.
How to conduct feedback:
The preloaded questionnaire link is activated 20 minutes before the session's end time.
- The feedback form is automatically linked to the participant’s most recent session. Please share this link in the Zoom chat after the session: https://iecho.org/feedback. Participants must be logged into iECHO to access the feedback form.
- Additionally, participants can access the feedback form on the participant group or schedule page after the session ends.
If you would like to gather feedback for a previously conducted session:
1. Select the program by choosing View All Programs.
2. Choose a participant group, then select the session for which you would like to collect feedback.
3. Click Copy Feedback Link and share it with participants using the quick email feature, or create an announcement requesting their feedback.

Best Practices:
- Keep 2–4 minutes for conducting the feedback toward the end of each ECHO session.
- Share the form link in the Zoom chat: https://iecho.org/feedback
- Continuously prompt participants and guide them to provide their feedback.
- Refresh the form page to view the number of recorded responses in a feedback form.
- If the participant has not logged in and accessed the form link, access will be restricted. Ensure that the participant is logged in before opening the form link to enable access.
